USMLE Step 1 Exam

United States Medical Licensing Examination (USMLE) is a three-step exam for medical licensure in the United States and is sponsored by the Federation of State Medical Boards (FSMB) and the National Board of Medical Examiners (NBME).
USMLE measures a physician's ability to apply knowledge, concepts and principles. USMLE helps the medical students in demonstrating the fundamental patient-centered skills, which are important in health and diseases. It constitutes the basis of safe and effective patient care.

USMLE Step 1

The Step 1 is the first in the series of the USMLE. The Step 1 directly assesses the basic science knowledge of medical students or graduates and its applications in clinical situations.
The content areas tested for Step 1 are Anatomy, Physiology, Biochemistry, Microbiology and Immunology, Pathology, Pharamacology, Behavioural Sciences and a few interdisciplinary topics such as nutrition, molecular & cell biology are also tested.

THE IDEAL CANDIDATE FOR USMLE

You need to be under one of these three categories to be eligible for the Step 1 examination

  • The minimum elgibility requirement to sit for the step 1 exam is completion of basic sciences (i.e. must have passed 1st and 2nd yr of medical school).
  • The candidate’s medical school must be listed on the World Directory of Medical Schools (WDMOS) and recognized by the Educational Commission for Foreign Medical Graduates (ECFMG).
  • A medical student officially enrolled in, or a graduate of, a US or Canadian medical school program leading to the MD degree that is accredited by the Liaison Committee on Medical Education (LCME)
  • A medical student officially enrolled in, or a graduate of, a medical school outside the United States and Canada and eligible for examination by the ECFMG – Indian MBBS students after completion their 2nd year
  • A medical student officially enrolled in, or a graduate of, a US medical school leading to the DO degree that is accredited by the American Osteopathic Association (AOA)

Exam Pattern for USMLE Step 1

The Step 1 exam contains approximately 280 multiple choice questions (MCQs). The exam is divided into 7 blocks of 60 minutes each and administered over a time span of 8 hours. Single best answers are the only format of questions that appear on the USMLE Step 1 exam.

  • The USMLE scores are only reported on the three-digit scale. The minimum passing score on the Step 1 is 192
  • The exams are conducted by appointments all year round at Prometric Testing Centers in the US and outside US
  • During the defined time candidates have to complete the whole items of each block
  • You may answer the items in any order
  • You can review your responses, and change answers also
  • If you take Step 1 and receive a passing score, you cannot retake the examination to improve your score. However if you fail in the exam USMLE program limits to six the total number of times an examinee can take the same Step or Step Component.
  • You cannot take the same examination more than three times within a 12-month period. Your fourth and subsequent attempts must be at least 12 months after your first attempt at that exam and at least six months after your most recent attempt at that exam.
  • Indian MBBS doctors have to apply for ECFMG Certification, so they have to pass all the USMLE Steps and Step Components within a 7 year period. If you do not pass all Steps and Step Components required for ECFMG Certification within a maximum of seven years, your earliest USMLE passing performance will no longer be valid for ECFMG Certification

STEP 1 USMLE TOPICS

The purpose of the Step 1 exam is to assess whether you understand and can apply the basic sciences to the practice of medicine, with a focus on principles and mechanisms of health, disease, and therapeutic interventions in medicine.It includes all Science subjects of MBBS 1st and 2nd year. The subjects are as follows:

Most of the questions examine your knowledge of basic facts and definitions. Major part of the questions deal with your ability to solve problems using basic scientific principles.
